A game‑changer in HIV prevention has arrived: lenacapavir (LEN). With up to 100% effectiveness and just two doses a year, LEN has the potential to transform HIV prevention, especially for key and vulnerable populations in low‑ and middle‑income countries, including men who have sex with men, transgender people, sex workers, people who inject drugs, and incarcerated people. But access remains dangerously limited. Current allocations through the Global Fund would reach only 15% of global need, and so far just 20,000 people have received LEN. Many countries are excluded altogether, including from future generic versions, due to restrictive voluntary licences. MSF calls on Gilead to reduce prices, expand supply to meet real global demand, and sell directly to MSF so this life‑saving prevention tool can reach those who need it most. MSF also urges governments to use all available legal measures to challenge monopolies and enable wider, affordable access to lenacapavir worldwide.
